Document / Forms in a Child Care Setting Related to Illness & Prevention

Take a look below, and you will see two common documents/forms that you would most likely see when you are in a child care environment that are related to illness prevention, including information about what they are, how they should be completed correctly, their purpose, and even the importance to THP requirements.

Immunization Form for Child Registrants 

What it is / The Purpose

  • The picture on the left is used by all child care centres to collect every individual child's immunization history or an up-to-date exemption status for all vaccine-preventable diseases under the watchful eye of TPH's Vaccine Preventable Diseases Program.
  • Its sole reason is to make sure that the child meets the immunization requirements or was given an approved exemption before returning to the centre, which stops the spread. 

Requirements for Completion 

  • Either Parents or guardians must fill in the necessary information: the Child's name, date of birth, address, doctor's name, and print a copy of the child's immunization record.
  • If a child does not take part because of medical or religious reasons, then a copy of the exemption form must be attached. For example, the Ministry's Statement of Medical Exemption or Statement of Conscience/Religious Belief. (Ontario 2025)
  • The centre holds onto this record to ensure the child's acceptance of the file and that all the up-to-date vaccines are given. (Toronto Public Health 2025)

TPH Relevance 

  • TPH commands that all child care centres collect all information before entering ( as soon as practical after). This is needed because children without required immunization can be eliminated during an outbreak or disease spread. ( Toronto Public Health 2025)
  • TPH review these records to make sure the submission in licensing or outbreak investigations.

TB Screening Form 

What it is / Purpose 

  • The form on the right and below is used by all child care workers, students, volunteers, or to document their immunization status, TB skin test, and health evaluation before they can begin to work. 
  • The sole reason is to protect children by making sure that staff are all up-to-date on immunization and healthy to interact with children. 

Requirements for Completion 

  • First, the person will fill  out their name, role, date of birth, immunization dates or their proof of immunity for different diseases, such as MMR, Varicella, Tdap, etc. ( Toronto Public Health 2025)
  • TB skin test results may be required within six months earlier employment, and if it returns positive, then medical clearance, like a chest x-ray, physician not) is needed. ( Toronto Public Health 2025)
  • All documents on medical exceptions must be documented and kept on file at all times. ( Toronto Public Health 2025)

TPH Relevance

  • Under the Child Care and Early Years Act (CCEYA), before heading back to work, all employees must undergo a health and immunization check as recommended by the local Medical Officer of Health. (Toronto Public Health 2024)
  • If the outbreak does happen, the staff who are unvaccinated may be informed to stay at home until the outbreak is over, as per TPH guidance. ( Toronto Public Health 2025)
